Stabling form
The official stabling form is on the bottom of the official entry form. This separate form is included as a courtesy for show managers who also want competitors to fill out a separate form to give to their stabling organizers.
The Web manager does not have the entry forms as MS Word documents.
Note: On entry form ... a competitor is NOT required to list Social Security Number/Federal ID Number. But if prize money is awarded, then at that time, it is the sole responsibility of the competitor to provide the competition secretary with accurate payee information.
